A Chinese Board of Inquiry has determined that one of the two, upper-stage engines of the Long March 3A launch vehicle failed to perform at its peak, resulting in the loss of the Palapa-D satellite. At fault was the rocket engine's gas generator which suffered a burn through, probably due to either icing or foreign matter in the liquid hydrogen injector. This resulted in the Indonesian satellite being deposited into a useless orbit. Plan are now underway by China to fit a special filter on the liquid hydrogen system that will block ice or other alien materials from passing into the injector. Additionally, to rid the rocket of any ice buildup, the liquid hydrogen cavity in the third stage engine will be purged prior to launch. The Long March 3A is expected to back in service later this year.
